Instructions for foreign citizens and citizens without croatian insurance interested in ablation or implant procedure at our institution.
If a patient who is a foreign citizen (or a citizens without croatian health insurance) is interestent in electrophysiology, ablation or implantation procedure in our institution, please contact us via email.
The email should include patient’s general data (name, surname, adress, date of birth), procedure that is indicated and doctor’s report or summary (based on which indication for the procedure was done).
If the insurance company is covering the expanses of the procedure, data required for the pro forma invoice are:
- Name of insurance company
- Correct address of insurance company
Based on this information, offer and pro-forma invoice can be sent to the patient or the insurance company.
