Please download the necessary consents, read them carefully and if possible complete them, and bring them with you when admitted to the hospital. After consulting with your physician, if you consent to the procedure you can sign them. For any ambiguities you can ask the medical staff at the Centre for Arrhythmia and Cardiac Pacing.
